the age for name change as an adult is age 14.
You can consult your state statutes if you are
avoiding using a lawyer. The name change
kit contains the instructions and the forms you
need to move the name change procedure into the
court and get it done. First you
have the "Name Change Petition/Application.
Fill this out completely, sign and have
notarized. Attached to the Petition will
be a certified copy of your birth certificate, a
notification list, copy of the notification
letter, your signed and notarized affidavit and
the application for name change. All of
these forms are included in the kit except your
birth certificate. The name change forms
should be filed in your state court of
jurisdiction and you can determine that by
calling one of the courts and give them your
address. We have a databank of additional
forms which are not required in most courts of
jurisdiction. If your court requires an
